Co-Payments and Deductibles

All patients must be prepared to make the necessary payment arrangements to cover any co-payments or deductibles as dictated by your insurance plan at the time of service.

Additionally, please be aware that you will be asked to make payment on any outstanding balances due at the time of service. We have professional billing staff in each of our medical offices to answer any questions you might have. For your convenience, we accept cash, check, MasterCard, and Visa.

Referrals & Authorizations

Please be aware that, according to the managed care organizations that we work with, all patients must obtain a referral from their primary care physician in order to see a specialist. In the past, we have been able to call your primary care physician to obtain your referral for you. However, due to the increase in the amount of HMO/POS patients we see on a daily basis, we can no longer do this on your behalf. It is solely the patient’s responsibility to secure the initial referral as well as keep up with the number of follow-up visits allowed and the expiration dates on your referrals. Please do not assume that your primary care physician has called or faxed our office with the necessary authorization. You must have it at the time of your appointment. Failure to present an authorization for services may result in having to reschedule your appointment.

The following represents the most current list of insurance carriers that are on our insurance panel. Due to the dynamic nature of our business and our relationships with insurance companies in Jacksonville, this list is subject to change without notice. It is important to note for our HMO/POS patients that, according to the insurance companies that we participate with, all patients must obtain a referral from their primary care physician in order to see one of our nephrologists. Its always a good idea to confirm whether or not we accept your insurance. The following list is meant to serve as a guide only and is not meant to be entirley inclusive or exclusive. Do not hesitate to call our office at 904-389-5333 to see if an unlisted insurance is now accepted.

You will be asked for your referral number when you call to make an appointment and you must have it at the time of your appointment. If you arrive for your routine appointment without your referral, it may be necessary to reschedule you for the next available date.
